Requesting Your ACA Form –

If you need a copy of your ACA Form, you can request it electronically by following these steps:

Step 1: Send an Email Request

Email [email protected] with the following details:

• Your full name
• The last four digits of your Social Security Number (SSN)

Step 2: Verification

Once we receive your request, we will verify your identity.

Step 3: Receive Your Form

Upon verification and with your consent, we will send your ACA form securely to your personal email.
